Please note you may see any clinician you wish to see for your consultation; although it is helpful if you can see the same clinician for an ongoing problem as this assists with the continuity of your treatment.

UPDATING YOUR MEDICAL RECORDS

We may be required to contact you from time to time to request that you attend the surgery for a blood pressure check, or to bring in a urine sample for testing. These are usually done annually and we would be grateful if you could respond to these requests. However, if you are housebound and unable to attend the surgery please let the receptionists know.

RECALLS FOR CHRONIC DISEASE CLINICS

Due to the increase in postage costs we have been asking patients to complete a mobile/Email consent form. This will enable us to contact patients either by text, telephone call or Email to remind them to make an appointment for their chronic disease review. If a blood test is required your blood test form will be available at the surgery for you to collect. You may have your review appointment first and collect your blood test forms at that time. You may contact reception any day after 2pm for your blood test results.
